외상 환자에서 말초삽입 중심정맥관의 유용성: 단일기관의 초기경험

Abstract

Purpose:

A peripherally inserted central catheter (PICC) provides effective, reliable intravenous access in patients who require long term therapy such as intravenous antibiotics, total parenteral nutrition, transfusion or inotropic agents. This retrospective study evaluated the usefulness of PICC in trauma patients by examining patient characteristics and common complications, including PICC related bloodstream infection.

Methods:

We reviewed the trauma patients who underwent PICC at Pusan National University Hospital Trauma Center from January 2016 to February 2017.

Results:

From January 2016 to February 2017, 32 patients underwent PICC. Total catheter insertion days were 875 days, and the average catheter indwelling time was 27.3±25.02 days. The most common indication for PICC was total parenteral nutrition (n=20, 62.5%), while the remainder was to ensure a long-term fluid administration route (n=12, 37.5%). Catheter related complications included infection (n=3, 9.4%; 3.42 per 1,000 catheter-days), catheter tip malposition (n=2, 6.3%), catheter dislodgement (n=1, 3.1%), insertion site leakage (n=1, 3.1%) and arm swelling (n=1, 3.1%). No statistically significant differences were found between those who developed bloodstream infection and those who did not.

Conclusion:

If the PICC is performed by correcting adjustable factors that increase the risk of infection, effective and reliable intravenous access can be maintained in patients who require long-term therapy without bleeding, pneumothorax, or other complications of central venous catheter insertion.

서론

중심정맥관(central venous catheter, CVC)은 고장성 수액, 혈액제재, 총 정맥영양과 항생제 같은 다양한 약물들의 안정적인 혈관 내 주입이 가능하며, 환자의 혈액검사와 활력징후 감시가 가능하여 입원환자 치료에 중요한 역할을 하고 있다. 말초 삽입 중심정맥관(peripherally inserted central catheter, PICC)은 말초정맥에서 접근하여 중심정맥에 위치하게 하는 특별한 형태의 중심정맥관으로, Hoshal [1]에 의해 도입된 후 현재 중환자실을 포함하여 장기간 CVC가 필요한 환자에서 널리 사용되고 있다. PICC의 삽입은 접근 경로로 자쪽 피부정맥(basilic vein) 또는 두부 정맥(cephalic vein)을 주로 사용함으로써, 전통적인 CVC 삽입 시에 일어날 수 있는 출혈, 기흉과 같은 시술과정에서의 합병증을 피할 수 있고 관리가 용이하며 환자의 순응도가 높은 것이 특징이다[2,3]. 중증외상 환자들은 장기간의 입원이 필요한 경우가 많아 안정적인 수액 공급 경로가 필요하고, 이에 따라 중증 외상 환자의 치료에 PICC의 사용이 늘어나고 있다. 전통적인 CVC에 비해 장기간 안정적으로 수액공급 경로를 확보할 수 있는 PICC 사용이 늘어남에 따라서 효율적인 시술방법과 장기간의 거치에 따른 관리, 그리고 혈류 감염을 포함한 합병증의 발생 예방에 대하여 다양한 연구가 이루어지고 있다[4,5].
저자들은 외상중환자실과 병동에서 PICC를 시술 받은 외상 환자의 후향적 연구를 통해서, PICC 시술과 유지, 혈류감염을 포함한 일반적인 합병증을 파악하여 외상 환자에서 PICC의 유용성을 알아보고자 한다.

대상 및 방법

본 연구는 2016년 1월부터 2017년 2월까지 부산대학교병원 권역외상센터에서 PICC를 시행 받은 총 32명의 외상 환자를 대상으로 하였다. 본 연구는 부산대학교병원 임상시험심사위원회의 승인을 획득하였다(IRB no. 1708-018-058). PICC를 시행한 환자들의 나이, 성별, 손상중증도(injury severity score, ISS), 시술 적응증, 사용혈관과 팔의 위치, 환자의 동반질환과 시술시의 혈액검사와 감염상태를 후향적으로 조사하였다. 또한 시술 시의 입원일수, 수술 후 일수, 중환자실 체류상태와 체류일수, 총 카테터 거치일수, 그리고 PICC로 인해 발생한 합병증을 조사하였다.
모든 PICC의 시술은 외상외과 의사 한 명에 의해 시행되었으며, 5 French dual-lumen 카테터를 사용하였다. 2% chlorhexidine gluconate를 포함한 무균적 예방조치를 시행하였고, micro introducer technique과 이동식 초음파를 이용하여 침상 곁(bedside)에서 시행하였다.
카테터 끝의 위치는 시술 후 흉부 단순 촬영으로 확인하였고 조정이 필요한 경우 위치를 조절하였으며, 시술 과정에서도 카테터가 동측 내경정맥 또는 반대측 쇄골하정맥으로 삽입되는 경우를 감시하였다. 모든 PICC의 관리는 2% chlorhexidine으로 일주일에 2회 드레싱하였고, 투명 드레싱 제재를 이용하여 삽입 부위를 감시하였다.
PICC 거치 기간은 삽입일로부터 제거일까지 계산하였고, 감염까지의 기간은 삽입일로부터 처음 혈액 세균배양검사에서 양성이 나온 날까지 계산하였다.
PICC를 시술 받은 환자가 발열이 있을 경우 카테터를 통하여 혈액 배양검사를 시행하였고, 균이 동정된 경우 PICC로 인한 혈류 감염으로 판단하고, PICC를 제거하면서 카테터의 끝의 배양검사를 실시하였다. PICC로 인한 혈류감염은 National Healthcare Safety Network surveillance definition에 따라 정의하였다[6].
통계적 분석은 IBM SPSS Statistics ver. 21.0 (IBM Co., Armonk, NY, USA)을 이용하여 two-tailed t-test with pooled variance 및 two-tailed Fisher’s exact test로 분석하였으며, p값이 0.05 미만일 때 통계적으로 유의한 것으로 간주하였다.

결 과

2016년 1월부터 2017년 2월까지 32명의 환자가 PICC를 시술 받았고, 총 카테터 삽입일수는 875일이었다. 남자가 대다수였고(n=27, 84.4%), 총 정맥영양이 PICC의 주요 적응증이었다(n=20, 62.5%). 중환자실에의 시술 빈도가 높았고(n=24, 75.0%), 자쪽 피부정맥(n=29, 90.6%)과 오른쪽 팔이(n=20, 62.5%) 주요 삽입경로로 사용되었다. PICC를 시술 받기 전 많은 환자들이 객담배양검사에서 양성소견을 보였고(n=21, 65.6%), 혈액배양검사와 상처배양검사에서 각각 3명(9.4%)과 5명(15.6%)이 양성이었다. 동반질환으로는 당뇨가 가장 많았고(n=6, 18.8%), 환자들의 평균 ISS는 23점이었으며, 대다수의 환자는 중증 외상 환자였다(n=29, 90.6%). PICC 시술 당시 환자들의 입원 일수는 평균 19.1±16.69일이었으며, 카테터의 평균 거치 기간은 27.3±25.02일이었다(Table 1).

32명의 환자 중 3명(9.4%, 1,000 카테터 일당 3.42건)의 환자에서 PICC로 인한 혈류감염이 발생하였고, 감염까지의 평균 기간은 18±8일이었다. PICC 시술 후 발열이 발생하여 PICC를 통하여 혈액배양검사를 실시하였고, 양성 소견을 나타내었다. 이후 카테터를 제거하였고, 카테터 끝의 혈액배양검사에서는 1명의 환자에서 동일한 균이 검출되었다. 감염이 발생한 환자들과 감염이 발생하지 않은 환자들을 비교하였으며, PICC 적응증 항목에서 p값이 0.04로 통계적으로 의미가 있는 것으로 보이나, 그 외의 항목에서는 의미 있는 차이점을 보여주지 못하였다(Table 2).

32명의 환자 중 3명(9.4%, 1,000 카테터 일당 3.42건)의 환자에서 PICC로 인한 혈류감염이 발생하였고, 감염까지의 평균 기간은 18±8일이었다. PICC 시술 후 발열이 발생하여 PICC를 통하여 혈액배양검사를 실시하였고, 양성 소견을 나타내었다. 이후 카테터를 제거하였고, 카테터 끝의 혈액배양검사에서는 1명의 환자에서 동일한 균이 검출되었다. 감염이 발생한 환자들과 감염이 발생하지 않은 환자들을 비교하였으며, PICC 적응증 항목에서 p값이 0.04로 통계적으로 의미가 있는 것으로 보이나, 그 외의 항목에서는 의미 있는 차이점을 보여주지 못하였다(Table 2).
2명의 환자에서 카테터 끝이 동측의 내경정맥으로 삽입되어 상대정맥으로 카테터를 위치하지 못하였고, 쇄골하정맥에 카터터 끝을 위치하게 하였다. 2명 중 한 명은 34일간 사용하였고, 다른 한 명은 치료과정에서 의도치 않게 3일만에 카테터가 제거 되었다(catheter dislodgment). 한 명의 환자에서 수액 누출이 발생해 카테터 막힘을 의심하여 9일만에 카테터를 제거하였고, 팔 부종이 발생한 환자도 정맥 혈전증을 의심하여 3일째 카테터를 제거하였다(Table 3).

감염이 발생한 3명의 환자는 모두 중증 외상 환자였으며, 장기적인 수액치료를 위해 PICC를 시술하였다. 한 명의 환자는 입원 25일째 일반병동에서 시술 받았고, 27일간 사용하였다. 나머지 2명의 환자는 입원 28일과 26일째 중환자실에서 시술 받았고, 17일과 11일째 제거되었다. 통계적인 의미는 없으나 중환자실에서 시술 받은 환자에서 시술로부터 감염까지의 기간이 더 짧았다(Table 4).

고찰

본 연구는 PICC를 시술 받은 환자의 특징과 합병증의 후향적 분석을 통해 외상 환자에서 PICC의 유용성을 알아보고자 하였다. PICC는 관리의 용이성과 환자의 높은 순응도 뿐만 아니라, 일반적인 CVC의 시술에서 발생할 수 있는 의인성 합병증을 피할 수 있어 그 사용이 점차 늘어나고 있다. 대부분의 연구에서 잘 훈련된 시술자에 의해 시행함으로써 시술과정에서의 합병증은 발생하지 않았으며, 방사선 투시 장비를 사용하지 않고 침상에서 시행하여도 높은 시술의 성공률을 보고하고 있다[7,8]. 본 연구에서 모든 PICC는 micro-introducer technique을 이용하여 침상에서 시행하였다. 비록 2명의 환자에서 상대정맥으로 카테터 끝을 위치하지 못하고 쇄골하정맥에 위치하였지만 모든 환자에서 성공적으로 카테터를 삽입하였다. 많은 연구들에서 침상에서의 시술의 높은 성공률과 적은 비용을 장점으로 보고하고 있다. 저자들은 환자의 대부분이 중환자실에서 시술을 받았으며(75.0%) 중환자의 이동은 잠재적으로 많은 위험요소를 가지고 있다는 점에서, 환자의 이동이 필요 없이 침상 곁에서 시행할 수 있는 PICC가 매우 큰 장점이라고 생각한다[9].
일반적으로 PICC로 인해 일어날 수 있는 합병증으로는 카테터 막힘, 카테터 빠짐, 정맥염, 혈전증과 혈류감염 등이 있다. 물리적인 자극이나 시술과정과 관리의 부주의로 정맥염이 발생할 수 있는데, 연구들에 의하면 3.8%에서 8.2%까지 보고되고 있다[5]. 카테터 막힘은 3%에서 18%까지 보고되고 있고[10], 본 연구에서도 한 명의 환자에서 카테터 막힘으로 인한 수액 누출이 발생하였다. PICC로 인한 상지정맥의 혈전증은 2.5%까지 발생하는 것으로 보고되고 있으나 페색전증과 같은 치명적인 합병증의 발생은 드문 것으로 보고되고 있다. 사용 혈관에 따라 혈전증의 빈도가 달라지며[11], 카테터의 굵기에 비례해서 빈도가 증가한다[12]. 따라서 카테터를 선택할 때 환자상태에 따른 의료진의 합리적인 판단이 필요하다. 본 연구에서는 한 명의 환자에서 팔 부종이 나타나 3일째 카테터를 제거하였다. 카테터 제거 후 혈전증의 감시를 위해 초음파검사와 컴퓨터 단층 촬영을 시행하였지만, 상지의 혈전이나 폐색전증은 발생하지 않았다.
중심정맥으로 인한 감염(central line-associated bloodstream infection, CLABSI)은 병원 내에서 CVC를 가지고 있는 환자에서 가장 중요한 합병증이며, 많은 연구들이 이루어지고 있다. 감염은 입원기간을 늘리고 병원 비용을 증가시키며, 환자의 생명에도 치명적인 영향을 줄 수 있다. 현재까지 PICC는 병원 내 중환자들 보다는 경증의 입원환자와 통원 환자들에게 많이 시행되었다. 따라서 PICC로 인한 병원 내와 중환자실에서의 감염위험도에 대한 많은 연구들이 이루어지지 않았다[13]. 일반적인 CVC에 비해서 PICC의 혈류감염 위험도가 낮다고 인식되어 왔으나, 최근 연구들에서 입원환자에 있어서 PICC가 다른 CVC와 비교하였을 때 감염의 위험도가 유사한 것으로 보고되고 있다[14-17]. PICC로 인한 혈류감염은 입원 기간, 중환자실 체류상태, 그리고 PICC의 내강의 개수 에 따라서 감염 빈도가 증가한다고 보고되고 있다[18]. 또 다른 연구에서는 심부전, 복강 내 오염, 클로스트리디움(Clostridium) 감염, 그리고 기관절개가 PICC에 의한 혈류감염을 증가시킨다고 보고하고 있다[19]. 다시 말해 급성기의 중증환자에서는 PICC를 피하는 것이 좋으며, 앞선 연구에서도 급성기의 치명적인 환자들에 대해서는 PICC 시행을 추천하지 않고 있다[18]. 그럼에도 불구하고 임상적으로 반드시 필요한 경우에는 PICC로 인한 혈류감염을 막기 위해 무균적 시술과정과 드레싱, 그리고 카테터의 적절한 선택이 필요하다[20].
기존의 연구에 따르면 CVC로 인한 혈류감염의 빈도는 카테터 삽입 1,000일당 2.1건에서 30.2건까지 다양하게 보고되고 있으며, 특히 외상중환자실에서는 5.8건으로 보고되고 있다[21]. 본 연구에서는 3명의 환자에서 PICC로 인한 혈류감염이 발생하였다. 총 카테터 삽입일수는 875일이었고, 감염 비율은 1,000일당 3.42건으로 기존의 연구들과 비슷한 결과를 나타내었다. 환자들은 평균적으로 입원 후 19일째 시술하였다. 입원 시에 활력징후가 안정되어 있고, 오랜 기간의 수액주입 경로가 필요할 것으로 판단되는 안정된 환자는 입원 초기에 시행하였지만 중증외상으로 활력징후가 불안정한 환자들은 소생(resuscitation)을 시행하고 안정된 상태를 확인한 후 PICC를 시행하였다. 또한 시술 전 객담, 혈액, 상처 배양검사에서 양성반응을 보이는 환자들 중에서 임상적으로 활동성 감염이 있다고 판단되는 경우에는 PICC를 삽입하지 않았다.
PICC로 인한 감염은 카테터의 원인, 시술자의 원인, 그리고 환자의 원인 등 다양한 요인들에 의해서 발생한다는 것을 인지하고, 이러한 원인들에 대하여 조정이 가능한 요인들을 제거한다면 PICC로 인한 혈류감염을 최소화하여 장기적으로 안전한 수액공급 경로를 확보할 수 있을 것이다.
본 연구의 첫 번째 한계점은 외상센터에 입원한 환자를 대상으로 실시한 PICC의 초기경험으로 대상 환자수와 전체 카테터 삽입일수가 적었다는 것이다. 합병증의 발생빈도는 기존의 문헌과 큰 차이가 나타나지 않았지만, 향후 더 많은 환자를 대상으로 하는 연구가 필요하다. 또한 이번 연구 에서는 감염 환자들과 비감염 환자들을 비교하였지만, 환자수가 적어 두 환자군 간에 통계적으로 유의한 차이점은 발견할 수 없었다. PICC의 삽입 적응증에서 통계적으로 유의한 차이를 보이기는 했으나, 그 수가 적어 임상적으로 큰 의미를 부여하기는 힘들다고 생각된다.
두 번째 한계점으로 다양한 환자군과 시술자로 인하여 본원의 외상센터에서 시행한 일반적인 CVC 삽입으로 인한 혈류 감염의 빈도와 비교 분석을 시행하지 못하였다. 앞으로 이에 대한 무작위 대조시험(randomized controlled trial)이 필요할 것으로 생각된다.
외상 환자들을 대상으로 한 본 연구에서 일반적으로 PICC 삽입으로 인해 일어날 수 있는 합병증들은 기존의 연구 결과와 비슷한 빈도로 발생하였다. PICC의 감염 위험도를 증가시키는 여러 가지 조정 가능한 요인들을 교정하여 PICC를 시행한다면, CVC의 시술과정에서 발생할 수 있는 합병증을 피하면서 오랜 기간 안정적인 수액공급 경로를 유지할 수 있을 것이다. 따라서 외상 환자에서 적절한 PICC의 사용은 치료에 도움이 될 것이라고 생각한다.
