63세 남자가 길을 걷다 넘어지면서 도로 연석에 상복부를 부딪힌 후 발생한 복부통증으로 응급실에 내원하였다. 내원 시 혈압은 140/70 mmHg, 맥박 100회/분, 호흡수 22회/분, 체온 36°C였으며 신체검사에서 우상복부 압통을 호소하였다. 응급실에서 시행한 외상응급 초음파(focused assessment sonography in trauma)에서 양성이었고, 복부 전산화단층촬영에서 담낭 파열에 의한 혈복강 소견이 관찰되어 응급개복술을 시행하였다(
Fig. 1). 수술 소견상 담낭 파열에 의한 혈복강 및 담즙누출이 관찰되었다(
Fig 2). 주위의 다른 손상은 발견되지 않아 담낭을 절제한 후 수술을 종료하였다. 복부 둔상으로 인한 담낭의 단독 손상은 극히 드문데[
1], 대부분의 사례는 오토바이 사고나 보행자 교통사고로 인해 발생한다[
2,
3]. 대부분 우상복부 통증을 호소하지만 증상이 없어 진단이 지연되는 경우도 있다[
4]. 담낭 손상의 표준 치료는 담낭절제술이다.
